To refer a child for services from The Faces of Hope, please click here to fill out the referral form. Our principal service is a one-year program of physical activity and nutrition education for children ages 6-18 years old. These children are in the 85th–95th percentile (at the risk of overweight) and the 95th+ (overweight) in terms of BMI (Body Mass Index).
What makes us unique? We work in the child's home environment and with the family. Our Support Team consists of a fitness trainer and nutritionist personally assigned to the child. We will develop a physical activity plan and nutritional plan, especially for the child, including their personal interest and cultural preferences. The Support Team will work one-on-one with the family to achieve the child's goals and report progress on a periodic basis throughout the program. We have various group and family activities and workshops planned throughout the year as well as various incentives for achievement of major milestones and goals. Join us for a fun and healthy journey!The Faces of Hope staff and referral resources are available for the family as well. Additionally we work closely with the child’s physician.
Other supportive services include workshops and consulting
activities related to developing a healthier lifestyle by improving
physical activities and nutrition education. We will conduct
workshops or consulting services for various organizations, youth
